Your existing gear is weak in other lands and you need to start collecting gear again. Items, quests, and enemies don't have levels, but rarities which are denoted by stars and color coded from white to green to blue to purple to yellow.
You need to keep your weapon and armor rarity in mind when attacking enemies or choosing your next quest. The main tool of your explorations is the voxel-based world map. It allows you to scroll and zoom anywhere in the world to help you find your next destination. It's up to you where to go next. You will learn to read the world map, find Shrines of Life , villages, towers, dungeons and more. NPCs will help you and mark quests on the map.
